How we’re supporting Ontario with development of Primary Care Networks (PCNs) in Ontario Health Teams (OHTs)
PCNs will connect, integrate, and support primary care professionals within OHTs to improve the delivery and coordination of care for patients.
PCNs have two core objectives:
To organize the local primary care sector in OHT planning and provide a voice in OHT decision-making.
To serve as a vehicle to support OHTs in the implementation of local and provincial priorities.
